PCPCH Program Tier 3 Star Recognition

As you may know, over 500 clinics in Oregon are recognized as Patient-Centered Primary Care Homes. A majority of these clinics are recognized at the highest level of the model, Tier 3. These figures represent significant achievement in primary care transformation work in our state over the past two years.

In an effort to recognize distinguished leaders in practice transformation and encourage continued transformation progress, the PCPCH program is developing a special designation for clinics who are trailblazers in practice transformation – Tier 3 Star. 

PCPCH Program Community Forums in Ontario and Pendleton

Announcements - September 19, 2013

The Oregon Health Authority Patient-Centered Primary Care Home (PCPCH) program is hosting two community forums in October, sponsored by the Eastern Oregon Coordinated Care Organization (EOCCO). Attendees will learn more about the PCPCH program, including the 2014 recognition criteria and how to become a recognized primary care home.

The Patient-Centered Primary Care Institute (PCPCI) is pleased to announce a new training opportunity: We are looking for 24 – 30 practices in Oregon to participate in a training series offered by a national expert on integrating behavioral health in primary care settings in October 2013.

Request for Proposal: Strategic Planning

The Institute is seeking a contractor to facilitate a strategic planning process to focus our direction and financial sustainability.  The contractor selected for this project will complete a strategic planning process to inform the Institute’s programming priorities and business model.  The timeframe for this work is July – October 2013.  Proposals are due June 24, 2013

Learning Collaborative Practices Selected

Announcements - January 4, 2013

The Patient-Centered Primary Care Institute has selected 25 practices from across Oregon to participate in its first Learning Collaborative, running from January 2013–October 2013. The Learning Collaborative will incorporate multiple learning methods to maximize opportunities for the selected practices to learn from each other and from technical experts in topic areas aligned with PCPCH Standards.
