"Transformation in Practice" is a webinar series hosted by the Transformation Center featuring clinics that have successfully implemented some of the most commonly misunderstood measures of the Patient-Centered Primary Care Home (PCPCH) model. During the webinars PCPCH program staff provide in depth technical assistance on the featured measures. Their expertise is based on conducting site visits to PCPCHs across the state. In addition, staff from a recognized PCPCH will present on how they have implemented the measure to transform their practice.
During the first webinar in this series, join PCPCH program staff and a clinic partner for this in-depth technical assistance webinar on implementing a clinic-wide quality improvement strategy to meet Patient-Centered Primary Care Home measure 2.D.3. PCPCH program staff will share best practices they have learned from visiting clinics across the state. Clinical staff from Oregon Medical Group Gardenway Medical Clinic in Eugene will share their Quality Improvement tool Daily Management System.

2.D.3 PCPCH has a documented clinic-wide improvement strategy with performance goals derived from community, patient, family, caregiver, and other team feedback, publicly reported measures, and areas for clinical and operational improvement identified by the practice. The strategy includes a quality improvement methodology, multiple improvement related projects, and feedback loops for spread of best practice.
