Institute Partners with CareOregon to Provide Practice Coach Training
The Institute has partnered with CareOregon and CEPC to provide logistical support for the PCPCT trainings offered in Portland.
Portland, Oregon -- Since 2013, the University of California San Francisco (UCSF) Center for Excellence in Primary Care (CEPC) and the Patient & Population Centered Primary Care program of CareOregon have partnered to teach the Practice Coaching for Primary Care Transformation (PCPCT) training course material in an effort to bring clarity to the world of transformation work. In alignment with organizational objectives, the Patient-Centered Primary Care Institute (PCPCI) has joined CareOregon to provide logistical support for the PCPCT trainings offered in the Portland area. The PCPCT course is an orientation to, and comprehensive review of, the science of change in primary care practice and equips participants with practical tools and coaching skills necessary to engage and support teams as they evolve into high performing Patient-Centered Medical Homes.
